Meal Acommodations

Does your student require a meal accommodation due to allergy or other reason? If so, please complete the Meals Accommodation Form (allergies will require physician signature) and return to the front office. Physician offices may also remit forms to the school via fax at 951-274-4231 or by emailing our Health Assistant at kspongberg@riversideunified.org
